Social Security Disability Forms

To receive benefits, you will be required to submit a number of Social Security Disability forms. Some must be filled out at the beginning of the process, while others may be necessary down the road if your claim gets denied.

For starters, you will need to complete an application and medical records release, as well as the Adult Disability Report, the Work History Report and the Function Report. If your claim gets denied, you will likely have to complete a number of other forms to file your appeal.

The amount of paperwork required by the Social Security Administration can be overwhelming. Some forms are fairly simple and can be completed quickly. Others are very long and may take hours, or even days, to finish. All told, you will be required to complete about 40 pages of forms and answer more than 130 questions during the application process alone.
