How does the Social Security Administration (SSA) decide who qualifies for Coronary Artery Disease Disability benefits?
People who suffer from coronary artery disease, also known as coronary heart disease or ischemic heart disease, may be declared disabled by the SSA during the application stage if they meet certain criteria. In order to get approved at the beginning of the Disability process, you must be experiencing symptoms of “myocardial ischemia” such as:
- Angina Pectoris ― chest discomfort that is brought on by activity or emotion and promptly relieved by rest or nitroglycerin tablets.
- Atypical Angina ― pain or discomfort in areas other than the chest, such as the inner arm, jaw, neck, back or upper abdomen.
- Anginal Equivalent ― shortness of breath but no chest pain or pressure.
- Variant Angina ― episodes of angina at rest caused by the spasm of a coronary artery.
- Silent Ischemia ― the occurrence of myocardial ischemia or infarction without any pain or symptoms.

In addition to the symptoms described above, the SSA requires that you also have one of the following ― an abnormal stress test, ischemic episodes, or abnormal imaging results.
Abnormal Stress Test – This is very technical, but a stress test is considered “abnormal” if you are exercising at a load that is equal or less than 5 METs and it causes one of the following:
- Horizontal or downsloping ST segment depression of at least -0.10 millivolts lasting at least a minute into the recovery phase.
- A minimum of 0.1 millivolt of ST segment evaluation during exercise lasting a minute or more into the recovery phase.
- A reduction of systolic blood pressure by 10 mm HG or more below the baseline pressure.
- Documentation of exercise level of 5 METs or less on a medically accepted imaging study such as an EKG.
Ischemic Episodes – You must have had three separate ischemic episodes within a consecutive 12-month period. Each episode must require revascularization, such as angioplasty or bypass surgery.
Abnormal Imaging Results – If taking an exercise tolerance test is too risky, the SSA will accept an angiography or other medically acceptable imaging device that shows coronary artery disease. The angiographic evidence must show 50 to 70 percent narrowing of a coronary artery that has not been bypassed. Your CAD must also severely limit your everyday activities.
